Diagnostic Imaging is home to the Ontario Breast Screening Program (OBSP)

Group Health Centre DI Department performs over 6,000 screenings per year as a public service through self-referral. You do not need to be a member of Group Health or have a Primary Care Provider to make an appointment.   The Ontario Government encourages women, Two-Spirit people, trans people and non-binary people ages 40 – 74 years book your mammogram.

  1. Players must be 18 years of age or older. Tickets must be ordered/purchased and delivered in the Province of Ontario.

    2. Tickets are purchased online at healthcentrelottery.ca, in person at the Group Health Centre with Trust Fund staff and/or volunteers, over the phone, or at Group Health Centre Trust Fund managed locations.

    3. The total tickets available will be 12,000 

    4. Ticket Prices: 

    • 1 ticket for $25 
    • 2 tickets for $45 
    • 4 tickets for $75

    5. Ticket sales will run from November 15, 2024, until February 5, 2025, at 3:30 p.m.

    6. Grand prize draws (1 – 3) will take place at 4:00 p.m. on February 5, 2025.

    7. There will be a total of 25 early bird draws. The first 13 draws, each for a $100 gift card, will take place on Friday, December 27, 2024, at 4:00 p.m. The remaining 12 draws, also for $100 gift cards, will take place on Friday, January 17, 2025, at 4:00 p.m. All tickets purchased by 3:59 p.m. on either of these dates will be eligible to win all subsequent draws including, including any remaining early birds and the three grand prize draws. Winning a previous draw does not disqualify you from winning another.

    8. If all 12,000 tickets are sold prior to February 5, 2025, at 3:30 p.m., all remaining draws (early bird and grand prize) will take place on the associated draw date with all eligible tickets.

    9. There is no limit to the number of tickets that can be purchased by any one individual, up to a maximum of 12,000 (maximum number of tickets available).

    10. Tickets are only valid from the moment of purchase and are not valid for any draws that occur prior to the purchase.

    11. Grand Prize 1 is $50,000 cash. Grand Prize 2 is $25,000 cash. Grand Prize 3 includes $2,500 in gift cards to any of our participating partners, in any denomination the winner chooses. The gift cards will be awarded to the first name that appears on the winning ticket and must be redeemed directly with each business. The gift cards cannot be redeemed for cash.

    12. The winner must provide their choice of business(es) and the amount(s) within 7 working days of being notified of their win. 

    13. All cash prizes are awarded free of any restrictions.  

    14. The winning ticket is chosen randomly through the BUMP 50:50 Electronic Raffle System. 

    15. A winning ticket holder does not need to be present at the time of the draw to win. 

    16. All winning ticket numbers will be posted to healthcentrelottery.ca following the draw and remain posted for a minimum of 30 days.

    17. By purchasing a ticket, you agree to allow us, The Group Health Centre Trust Fund, and its licensed gaming suppliers providing services to this program, the right, if you are a prize winner, to use and publicly disclose your name, municipality of residence, image, photograph and likeness as a prize winner in any means required by the terms of our lottery license and/or that we consider appropriate for purposes of administering and promoting the lottery, without notice, review or approval or additional compensation to you.

    18. The winner must provide his/her name, address, and phone number with picture identification for validation to claim their prize.  The identification must clearly verify the winner’s name, address, and age as corresponding to the information provided at the time of ticket purchase. Acceptable forms of identification include, but are not limited to, a driver’s license, passport, or provincial photo ID card. This requirement is in place to safeguard against fraudulent claims and to ensure that the rightful ticket holder receives the prize. Should the winner be unable to provide suitable identification within the allowed timeframe, or if the information does not match the details on the winning ticket, the prize claim may be subject to review or delay. 

    19. Prizes will be awarded to the primary ticket holder (the first name that appears on the ticket). In the event of a group win, in trust win or any win involving multiple individuals, any transfer of prizes must happen independently of the Group Health Centre Trust Fund.

    20. Winners will be notified via the telephone number provided at point of sale, or through email if an email address was collected.  The Trust Fund will continue to attempt to contact the winner for a period of 6 months.

    21. Once the certified winner is verified and provides identification with a photo ID, they will be given a cheque in the amount of the declared prize, in Canadian Funds (for cash prizes). All cash prizes will be delivered to the winner or sent via a registered trackable mail service.

    22. Early bird and Grand Prize winners will be required to sign off on the prize verification forms either in-person, via fax or digitally prior to receiving their winnings.  Prizes will be sent to the winner via a registered trackable mail service.

    23. The following individuals and their spouses, children and children-in-law are prohibited from ordering/purchasing a ticket or winning a prize: Group Health Centre Trust Fund employees, Group Health Association Board of Directors, and the Group Health Centre Executive Team.

    24. All prizes will be awarded as described in the rules of play. In the event that a partnered business is no longer able to offer a gift card to their business, the winner will be paid in Canadian funds equal to the value of the described prize.

    25. Volunteer ticket sellers are eligible to play, win prize(s) and otherwise participate in the lottery, providing their tickets are not purchased during their scheduled “shift” at the Group Health Centre Trust Fund managed location(s) and the transaction is completed by another volunteer or Group Health Centre employee. 

    26. Unclaimed prizes are subject to AGCO unclaimed prize policy and procedures. Any unclaimed prizes will be held in trust by the licensee for a period of six months from the date of the draw.
