Group Health Centre depends on donations and community support to run specialty programs – many of which are open to all residents of the Algoma district, not just patients of the Centre. These important programs help patients manage chronic conditions, ensuring they don’t end up in hospital or that their stays are shortened. Often underfunded, these programs need your support to be here for those who need it. These programs include:
Algoma Diabetes Education and Care
A team of educators, dietitians, nurses, a social worker, chiropodist, foot care nurse and primary care providers assist both adults and children with Type 1, Type 2, Gestational Diabetes and those with Prediabetes. To learn more, click here.
Algoma Respiratory Education Program
A program that supports people of the Algoma District who have, or are at risk of developing, respiratory problems through screening, assessment, treatment, education and support. Patients can include those who have asthma, chronic obstructive pulmonary disease (COPD) or who need screening for lung disease. To learn more, click here.
Cardiac Rehab Program
Exercise, education and counselling for patients who have had a "cardiac event" in the past year. To learn more, click here.
Child and Youth Mental Health Program (CYMHP)
A community program that provides child psychiatry and mental health services to children and youth under the age of 18 - including mental health assessments, medication review, short-term treatment, liaising with external agencies, discharge planning and care for the mental health & wellness of children and youth. Chiropody (Foot Care)
Examination, treatment, and education regarding the prevention of disease and conditions of the feet. This service works closely with the Algoma Diabetes Education and Care Program to provide Diabetic footcare among other services. To learn more click here.
Congestive Heart Failure Program (CHF)
Assists patients at risk for Congestive Heart Failure at home and over the phone with medication, diet and exercise. To learn more, click here.
Family Health Worker (FHW)
Provides holistic support, case management and advocacy for patients living with chronic disease, and their families. FHWs also provide follow-up of early discharge patients from obstetrics, surgery, heart conditions as well as support in the areas of oncology, bereavement and palliative/end-of-life care, helping keep many patients in their homes and out of hospital.
Phototherapy
A process that uses artificial light to treat psoriasis and other skin conditions. To learn more, click here.
